As reported in H2R Minutes (April 1): “Modern Healthcare looks at how a growing number of health systems, ACOs and medical homes are deploying care coordinators…but there are challenges, both cultural and financial.”

While the NY Times offered a cautionary tale about understanding who is coordinating what aspects of care (“They’re all just trying to help”, April 14, 2015), it’s undeniable that care coordinators – particularly certified care managers – have a role to play in today’s complex healthcare models.
